Scope: State of New York, Private Work.
Time Constraints: None. This notice is not required
by this state (See Important, below). Send any time after the
start of furnishing labor, materials or equipment. However,
while this document is not required it can be very useful as
a source of information for your Mechanic’s Lien claims.
When you feel you need this information, send
the notice immediately by Certified Mail to the owner of the
job. If the owner or his agent fails to provide this information
within 30 days of your demand, or falsely states the information,
and you cannot collect from the general or subcontractor after
a judgment, the owner shall be liable for the loss sustained
as though you supplied directly to the owner.

of all of the forms should be send to the owner and general
contractor, if any.
